Dr. James Fretwell

MS,MD,FAAFP Diplomate of the American Board of Clinical Lipidology, Diplomate of the American Board of obesity Medicine.

Dr. Fretwell has been practicing family medicine in Colorado since 1997.

His practice focuses on primary care, preventative care, cholesterol disorders and the treatment of obesity. After earning a Bachelor's degree in Biology at Walsh College in Canton, Ohio, Dr. Fretwell attended The University of Akron and received a Master's degree in Biology before continuing on to medical school. He received his Doctoral degree in Medicine in 1994 from The Northeastern Ohio Universities College of Medicine. Dr. Fretwell completed his residency in Family Medicine at Barberton Citizens Hospital in Barberton, Ohio. 

Dr. Fretwell is a member of the Colorado Medical Society, and Board Certified with the American Board of Family Medicine. He serves as an Associate Clinical Professor with The University of Colorado School of Medicine, He is on the Medical Staff at Longmont United Hospital and UCHealth Longs Peak Hospital. He is a lipid Specialist, and Diplomate of the American Board of Clinical Lipidology. He is a Fellow of the American Academy of Family Physicians. He is a Diplomate of the American Board of Obesity Medicine.

Dr. Brian R. Hughes


After completing his residency in 2003, Dr. Hughes began his private medical practice at Rocky Mountain Family Practice.

A graduate of Michigan State University with a Bachelor of Science degree in Zoology, Dr. Hughes attended Midwestern University Chicago College of Osteopathic Medicine, graduating in 2000.

Dr. Hughes is a member of the Colorado Medical Association, the American Osteopathic Association, and has been Board Certified with the American Osteopathic Board of Family Physicians since 2003.

In addition to his practice in Longmont, Dr. Hughes has been the Medical Director at Lifecare Center of Longmont since May 2005.  He is on staff at Longmont United Hospital.

Dr. Cooper


Dr. Paul Cooper has been board-certified with the American Board of Family Practice since 2000.

He joined Rocky Mountain Family Practice in July 2000 after completing his three year residency at Sparrow Hospital/University of Michigan Family Practice Residency Program in Lansing. 

He began his medical training at St. Georges University School of Medicine, Grenada, West Indies in 1993 and graduating in 1997.  His third and fourth year medical training rotations were completed through the University of California in San Francisco.

Dr. Cooper is a member of the Boulder County Medical Society and the American Medical Association.  He is on staff at Longmont United Hospital.

Dr Mast earned his Bachelor of Arts from The University of Colorado in Boulder and then his Doctorate in Osteopathic Medicine.

He completed his Residency in Family Medicine at the Northern Colorado Family Medicine Residency Program in Greeley, CO. He is a Board Certified member of the American Academy of Family Physicians.

Diane P. Smith, NP


Diane, as a Family Practice Nurse Practitioner, provides a wide range of primary and acute health care services to individuals of all ages.

As a nurse practitioner for 10 years, Diane has focused on patients' conditions as well as the effects of illness on the lives of her patients and their families. Making prevention, wellness and patient education her priorities, she encourages patients to participate in decisions central to their care. Prior to being a NP, Diane managed a Neuroscience/Orthopedic/Rehab hospital unit and was an assistant professor of Nursing.

Diane received her Bachelor of Science degree in nursing from the College of St. Teresa in Winona, Minnesota, a Master's of Science degree in Nursing from Loyola University, Chicago, Illinois, and a Nurse Practitioner Post-Master's Certificate in Family Practice from Northern Illinois University. Having raised five children, she lives with her husband in Longmont.
